This folder is designed to be a tool to prompt and assist families in preparing for the future. It is important to plan for the future for our family members with disabilities when the parents, siblings and/or primary caregivers are not able to provide critical information. There are questions about healthcare needs, residential plans, legal or financial planning, and community involvement that need to be addressed and recorded. Sharing these plans with relatives or close friends is helpful too.
