Eligibility

Becoming eligible for assistance through the Social Security Administration’s
disability programs is a process. You will need several forms of information so that Social Security can determine your eligibility. The information in this website will provide you with valuable information on applying for benefits.

How to apply

You may apply for benefits by:

Information You Need:

You will need to gather several types of information when you apply for benefits.  Below is a list of things you will need (some or all of this list may apply to you).

A summary of your work history, if applicable;

Your latest property tax bills (if a home or other property is owned) or copies of your lease or rental agreements;

Copies of, or proof of, utility and food expenses;

Payroll stubs;

Insurance policies;

Information on your bank accounts and other documents showing your resources or assets;

Special records or the names of medical personnel and/or facilities where you have received treatment or services;

Names, addresses and telephone numbers of all treating physicians and the dates of treatments;

Names, addresses, telephone numbers and records of clinics and hospitals and the dates of treatments.

Non-physician healthcare professionals: dates of physical and occupational therapy with names and addresses of treatment centers;

Information about your parent’s or spouse’s incomes if you live with your parents or are married;

Names, addresses and records of the schools you attended. The names of the most recent teachers and/or counselors;

Vocational Rehabilitation Services records and the names of vocational rehabilitation counselors. Dates of testing, evaluations and training programs;

Statements by the claimants, relatives, and friends, including the names, addresses and telephone numbers of those who knows about your condition and can give additional information;

Your Social Security Number (SS#) and the Social Security number of your spouse and dependents;

Original legal documents such as marriage certificates, divorce papers, birth certificates and adoption papers;

Names of banks and credit unions with checking and savings accounts information

Other information on benefits you receive such as, Veterans Assistance, Military Pensions, Unemployment Compensation, TANF, WIC, Medicaid, Energy Assistance, Workers Compensation, Food Stamps, etc.

You should ensure the information requested by the Social Security Administration is provided to them in a timely manner. This will help them process information in an efficient manner.
