Please help us plan for your arrival by pre-registering. If you are unable to do so, we will accept your registration on-site. Pre-registration closes at midnight on Sunday, September 19. If you have not already registered by that date, you may register on-site beginning on Wednesday, September 22.

Fees:

  
Forum Registration Fee:            $165.00 through August 24, 2010       $195.00 beginning August 25, 2010

Registration Fee Includes:


•    Attendance at all Forum sessions
•    Receipt of Forum materials
•    Breakfasts, lunches and refreshment breaks
•    Reception with cash bar (Wednesday, September 22nd)
•    Forum memento

Registrations Accepted: (credit card, check, money order of department billing)

•    On the payment screen, select:  Pay by fax or mail

Changes to Registration:


After you have completed your on-line registration, you may return to the form at any time to make changes by:

•    Clicking on the ‘login’ tab in the upper left corner of the registration form.
•    Entering your user name and password (if you have forgotten your information, the system will forward the         333information to your e-mail address.                           
•    Making changes/revisions

Cancellation of Registration:


A refund, less a $20.00 administrative charge, will be accepted if received by Sunday, September 19th. Refunds will be made in the same form that you submitted your registration ( i.e. if you paid your original registration with a check, your refund will issued in a check). After Sunday, September 19th, no refunds will be given.

All cancellation requests must be made in writing and either emailed or faxed to the Conference Registrar at:  iuconfs@indiana.edu or Fax: 812.855.8077
