Community Resource Map

Find useful transition resources for local and state organizations and services. Use the map by:

1. Typing or selecting a county or service category from the drop down menus in the blue box on the right.
2. Pins will appear that match your criteria.
3. Scroll down or click the Jump to List button for a list of local and state resources.
4. Zoom in or out by using the bar on the left of the map.
5. You may submit additional organizations or edit existing organizations for the map using this form
